Youth Take Charge Program - Participant Survey
You recently participated in a project funded by the Youth Take Charge Program at the Department of Canadian Heritage and are asked to complete a post-participation questionnaire. The Youth Take Charge Program (YTC) aims to strengthen youth attachment to Canada through engagement. The data collected will be used by Canadian Heritage to evaluate the impacts of the Youth Take Charge Program and improve the program for future participants. The questions are related to age and other demographic data, as well as respondents’ appreciation of the projects in which they participated. Your participation is voluntary and the estimated time to complete the questionnaire is 5 minutes.
Privacy Notice
The collection of personal information is authorized by
Subsection 5(a) of the
Department of Canadian Heritage Act
and is required to conduct the survey for the Youth Take Charge Program, which aims to evaluate program effectiveness and produce statistical reports on results. Collection and use of this personal information are in accordance with the
Privacy Act
. The information collected will be used for analysis, statistical and reporting purposes
